Continuum of Care

The Central Minnesota Continuum of Care (CoC) is a network of organizations, community residents and businesses that plan programs with the primary goal of alleviating homelessness in 13 counties in Central Minnesota, as well as the City of St. Cloud. The counties include Cass, Crow Wing, Todd, Morrison, Mille Lacs, Kanabec, Pine, Isanti, Chisago, Stearns, Benton, Sherburne, and Wright​.


Coordinated Entry

Coordinated Entry is a process that allows organizations throughout Central Minnesota to share a common intake and assessment tool in order to better coordinate our efforts to address the needs of the homeless. This allows agencies working with people experiencing homelessness to make consistent, effective, and efficient service matches throughout the Continuum of Care region, creating a "no wrong door" network in which those in need can be matched with appropriate services regardless of the organization at which they initially sought assistance. In other words, Coordinated Entry allows service providers to move beyond asking "what can my organization do for this person?" to instead "what service plan, using all the resources available in the community, can best meet this person's needs?"
